Terms of Use for Prospective Nurse Contractors

Welcome to Complete Infusion Services. Thank you for your interest in joining our team. These Terms of Use govern the application process and the subsequent engagement of nurses under contractor (1099) terms at Complete Infusion Services.

 

Application and Engagement Process:

  1. Eligibility: By applying for a position as an infusion nurse contractor, you confirm that you meet the minimum qualifications required for the role, including education, licensure, and experience, as specified in the job listing.

  2. Submission of Application: You agree to provide accurate and complete information during the application process. Any falsification, misrepresentation, or omission of data may result in the rejection of your application or termination of your contract if discovered after engagement.

  3. Confidentiality: During the application process, you may receive confidential information about Complete Infusion Services. You agree to maintain the confidentiality of all such information and to use it only for the purpose of your application.

 

Contractor Status:

  1. Independent Contractor Relationship: As an infusion nurse contractor, you are not an employee of Complete Infusion Services. You are responsible for paying your own taxes and for obtaining any required insurance, including liability and health insurance.

  2. Compliance: You agree to comply with all relevant laws and regulations governing your work, including HIPAA regulations for the protection of patient health information.

 

Conduct and Commitment:

  1. Professionalism: You agree to perform your duties professionally and ethically, providing high-quality care in accordance with the standards set forth by Complete Infusion Services and the relevant professional and regulatory bodies.

  2. Training and Compliance: You are required to complete any training programs provided by Complete Infusion Services and to comply with all operational guidelines, safety procedures, and privacy policies.

 

Intellectual Property:

  1. Use of Materials: Any training materials, documents, or other intellectual property provided during your engagement are the property of Complete Infusion Services. You agree to use these materials only in the manner intended and not to reproduce, share, or distribute them without explicit written permission.

 

Termination:

  1. Contract End: The terms of your contract will be specified separately in your contractor agreement. Either party may terminate the agreement according to the terms and conditions set forth therein.

 

Dispute Resolution:

  1. Mediation and Arbitration: Any disputes arising under these Terms of Use will first be tried through mediation. If unresolved, disputes will be settled through arbitration, not through litigation.
